Advances in Psoriasis Treatment

چکیده
The treatment of psoriasis is changing, thanks to a growing understanding that it is a disease of systemic inflammation.1,2 In this regard, drug candidates targeting the interleukin-23 (IL-23) and type-17 helper (IL-23/Th17) T-cell pathway appear particularly promising, offering high rates of clearance or near-clearance of psoriatic lesions.3–5 Along with their role in treating psoriasis, ustekinumab (Stelara, Janssen) and the tumor necrosis factor-alpha (TNF-a) inhibitors, in particular, are helping physicians target cardiovascular disease,6–8 one of the many known comorbidities of psoriasis. In the pathogenesis of psoriasis, the IL-23/Th17 pathway has been shown to be crucial in the immune system. Andrew Blauvelt, MD, MBA, president of the Oregon Medical Research Center, explained during a telephone interview with P&T:

Advances in Treatment Options for Psoriasis
Psoriasis is a skin disease that is evidenced primarily by plaques which can be seen throughout the body. Genetic predisposition that combines with an environmental trigger of the immune system is believed to be the root cause of psoriasis. This leads to signaling factors which coordinate the progression of inflammation and psoriatic plaques. Psoriatic patients commonly use topical agents, phot...
متن کاملRecent advances in phototherapy for psoriasis
Phototherapy involves repeated exposure of the skin to ultraviolet light to treat various inflammatory skin conditions such as psoriasis. Recent studies have identified specific immunologic effects of phototherapy that may underlie phototherapy efficacy. Furthermore, recent advancements have been made in developing safe and effective targeted phototherapy modalities for difficult-to-treat areas...
متن کاملMolecular Mechanisms and Management of a Cutaneous Inflammatory Disorder: Psoriasis
Psoriasis is a complex chronic inflammatory cutaneous disorder. To date, robust molecular mechanisms of psoriasis have been reported. Among diverse aberrant immunopathogenetic mechanisms, the current model emphasizes the role of Th1 and the IL-23/Th17 axis, skin-resident immune cells and major signal transduction pathways involved in psoriasis. The multiple genetic risk loci for psoriasis have ...
متن کاملDoes Short-Term Low-Dose Methotrexate Treatment Affect Homocysteine Blood Level in Patients with Psoriasis?
Background: An elevated homocysteine level is an independent risk factor for cardiovascular disorders. Psoriatic patients have an increased risk of cardiovascular diseases; In addition, hyperhomocysteinemia is a complication of methotrexate treatment. We undertook a study to evaluate the plasma levels of homocysteine, vitamin B12 and folate in patients with psoriasis before and after short-term...
متن کاملSerum vascular endothelial growth factor in Iranian patients with moderate-severe psoriasis before and after treatment: a PASI-75 response as a practical treatment goal
Background: Psoriasis is a chronic disease with multiple biochemical and vascular abnormalities. Several studies have evaluated circulating levels of vascular endothelial growth factor (VEGF) in psoriasis, but none of them evaluated it after reaching a PASI-75 response, as a practical treatment goal. The aim of this study was to evaluate serum levels of VEGF in moderate to severe psoriatic pati...
